We’re pleased to announce that we’re part of an innovative new initiative called Health Care Home, which is delivering a better experience in primary care. As part of this programme we’ll be making changes to the way we work, so that we can provide the flexible care that much of our population wants, while also being able to support those patients who live with long term conditions every day.

These changes will be happening over the next year, with some already starting now.

What does this mean for you?

We’ll be sharing more information about specific changes (remember to subscribe to our newsletter), but in the mean time here are some highlights:

We want to make it easier for you to get in touch with us

We’re upgrading our phone systems so you can get through to us on the phone.   We also have ManageMyHealth, so that you can work with us about your health online.

We’re also going to make it possible to communicate with your GP directly, both on the phone and online.  We may be able to save you a trip in to see us if you don’t need to, which will save you time and hassle.

We also want to make the most of your appointments

We know that your time is valuable, and so we want to use your time effectively.  We might contact you before your appointment and ask you to have some tests done (for example a blood or urine test) before your visit, so that we have more information before you arrive for your appointment.

We’re making our practice an even nicer place to visit

We’re always trying to make your appointment as simple and stress-free as possible, so we’re creating a quieter and even more comfortable waiting room.

We’re expanding our services

We’re able to provide more urgent care services, so that you can be fully treated here without need to travel to ED and wait to be seen.

We want to make it easier to live with a long term condition

Many of our patients have ongoing health conditions, and we want to make it easier to plan and manage your own health care so that you can maintain a high quality of life. We will work with you to set goals around your health and wellbeing, and work with you on a regular basis to achieve these goals.

We’ll also enable the time that you need to manage your conditions in consultation with us.

We’re working towards better hospital & after hours care

If you do need to go to hospital or after hours, they will be able to see some of your health information, allowing them to provide better care which is more personalised for you.

We’re excited about bringing you a refined primary care practice which can be the centre of your health care needs.
